Whitening for Life includes

• A one time fee of $295 (Normally priced at $375 without the program)
• Includes custom trays and whitening syringe
• Will receive a touch-up kit at every hygiene re-care visit (every 6 months)

How do I qualify?

• Must be at least 18 and not pregnant or nursing
• Must complete a full hygiene cleaning, exam & x-rays appointment
• Must have another hygiene appointment scheduled & kept (Note - For patients without insurance, hygiene appointment will be discounted)
• Must complete required dental care necessary to qualify for whitening
• Must not have any outstanding bills
• Must have a 6-month patient history without any broken appointments
